Join Our Team as a Care Coordinator

We are seeking a compassionate and proactive Care Coordinator to provide person-centered care and support to our service users. As a key member of our team, you will work collaboratively with our Extra Care Registered Manager, Deputy Manager, and other stakeholders to ensure the highest standards of care are provided.

About the Role

This is an exciting opportunity to join our well-established and experienced management team at Creative Support. As a Care Coordinator, you will be responsible for providing support with personal care and domestic tasks, and supporting individuals to access social opportunities. You will also be expected to work flexibly, including early and late shifts and weekend work on a rota basis.

Key Responsibilities
  • Provide person-centered care and support to service users
  • Work collaboratively with the Extra Care Registered Manager, Deputy Manager, and other stakeholders
  • Support individuals to access social opportunities
  • Work flexibly, including early and late shifts and weekend work on a rota basis
Requirements

To be successful in this role, you will need to have previous experience in services for older people, people with dementia and memory loss. You must also be willing to embrace all aspects of the role, including providing support with personal care and domestic tasks, and supporting individuals to access social opportunities.
