We are seeking a passionate and committed individual to join our team as a Scheme Extra Care Coordinator. As a key member of our team, you will play a vital role in empowering individuals to improve their quality of life and realise their hopes, dreams, and aspirations.

Key Responsibilities:

  • Support individuals with physical disabilities and learning disabilities to achieve their goals and aspirations
  • Develop and implement person-centred support plans to meet the unique needs of each individual
  • Work collaboratively with colleagues and external agencies to ensure seamless support
  • Contribute to the development of a positive and inclusive team culture
