Creative Support is a not-for-profit organisation that provides high-quality, person-centred support to people with physical disabilities, learning disabilities, and other care needs. We are seeking a dedicated and compassionate Care Coordinator to join our team.

As a Care Coordinator, you will play a vital role in empowering individuals to improve their quality of life, realise their hopes and dreams, and achieve their full potential. You will work closely with our team to develop and implement person-centred support plans, ensuring that each individual receives the support they need to live independently and with dignity.

We are looking for someone who is passionate about making a real difference in people's lives, and who is committed to delivering high-quality, person-centred support. If you have a strong understanding of the needs of people with care needs, and a willingness to learn and develop your skills, we would love to hear from you.

Key Responsibilities:

  • Develop and implement person-centred support plans
  • Work closely with individuals to understand their needs and preferences
  • Provide support and guidance to individuals to help them achieve their goals
  • Collaborate with our team to ensure that individuals receive high-quality, person-centred support
