Job Description:

  • Work with young adults aged 17, providing care and support in the community.
  • Implement the 3 Conversation Model to support young adults in their transition into adulthood.
  • Arrange college provisions, day opportunities, and supported living placements.

Requirements:

  • Degree level qualification in Social Work or equivalent.
  • Experience working with young adults with SEND.
  • Excellent communication skills and a passion for delivering high-quality care services.

Salary: £35 per hour

Working Hours: Flexible working arrangements available, including part-time hours.

Training Opportunities: Extensive in-house training opportunities available.
