About the Role:
As a Project Worker, you will be responsible for providing vital support to clients with mental health challenges, intellectual disabilities, autism, and addiction. You will work closely with healthcare professionals to develop personalized care plans, providing emotional support and guidance to clients and their families. Your ability to build trust and rapport with clients will be essential in helping them achieve their goals and aspirations.

Key Responsibilities:
• Develop and implement individualized care plans with clients
• Conduct regular assessments and reviews to monitor progress
• Provide emotional support and guidance to clients and their families
• Assist with daily tasks and activities to promote independence
• Participate in ongoing training and professional development to enhance skills and knowledge
