Job Title: Health & Social Care Support Worker

Job Summary:

We are seeking a highly motivated and compassionate Health & Social Care Support Worker to join our Reablement Team in Dumfries and Galloway. As a key member of our team, you will play a vital role in supporting individuals to maintain their health, independence, and wellbeing in their own homes.

Key Responsibilities:

  • Provide personal care and support to individuals, promoting their physical and emotional wellbeing.
  • Assist with basic household tasks and practical day-to-day activities to ensure individuals' homes are safe and comfortable.
  • Work collaboratively with the Reablement Team to develop and implement personalized support plans.
  • Communicate effectively with individuals, families, and other healthcare professionals to ensure seamless care delivery.
  • Contribute to the development and maintenance of a positive and supportive team environment.

Requirements:

  • A current driving licence (or alternative transport arrangements) is essential for this role.
  • Ability to work a shift pattern, including breakfast, lunch, tea, and bed visits.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassion, empathy, and a commitment to delivering high-quality care.
