Job Role

As a Home Care Assistant Coordinator with Routes Healthcare, you will play a vital role in delivering high-quality care to our clients. Your primary responsibilities will include:

  • Coordinating care plans with our multidisciplinary team
  • Providing direct care and support to clients
  • Maintaining accurate records and reports
