About the Role

We are seeking a compassionate and dedicated individual to join our award-winning team as a Team Leader at Avery Healthcare.

As a Team Leader, you will play a key role in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

Your focus will be on supporting the home management team in implementing, monitoring, and undertaking monthly evaluations of care plans, completing care plan audits, developing action plans, and being responsible for completing the action plans.

You will also be responsible for assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.

Additionally, you will lead the monitoring of the well-being of each resident, being aware of any change impacting care and care planning, and ensuring that changing needs are met and written records are amended to reflect any variation.

Practicing safe systems of work across a range of tasks and moving and handling of residents, you will assess risks and have due regard for personal safety and the safety of residents, visitors, and staff.

About You

Our ideal candidate must have an NVQ 3 or equivalent in care for care homes, two years' experience working in a senior position with the resident group, the ability to communicate effectively both verbally and in writing, and the ability to lead staff and act responsibly in the absence of the manager.
