Job Title: Care Practitioner Learning Disabilities Specialist
Job Summary: We are seeking a skilled Care Practitioner to join our Out of Hours District Nursing Team. The successful candidate will provide high-quality care to patients in their own homes, working to the Mersey Care values.
Main Responsibilities:
- Deliver evidence-based care to patients in their own homes.
- Work effectively as part of a team to achieve the best possible outcomes for patients.
- Develop and maintain strong relationships with patients, families, and carers.
Requirements:
- Registered Practitioner qualification.
- Mentor/teaching qualification or equivalent work-based experience.
- Evidence of clinical experience in a relevant field.
