Job Title: Care Practitioner Learning Disabilities Specialist

Job Summary: We are seeking a skilled Care Practitioner to join our Out of Hours District Nursing Team. The successful candidate will provide high-quality care to patients in their own homes, working to the Mersey Care values.

Main Responsibilities:

  • Deliver evidence-based care to patients in their own homes.
  • Work effectively as part of a team to achieve the best possible outcomes for patients.
  • Develop and maintain strong relationships with patients, families, and carers.

Requirements:

  • Registered Practitioner qualification.
  • Mentor/teaching qualification or equivalent work-based experience.
  • Evidence of clinical experience in a relevant field.
