As a Community Care Specialist, you will play a vital role in supporting patients to return home following a hospital admission and facilitating early supported discharge from hospital through access to health and care needs assessment at home.
The Home First Team service in Thanet, East Kent is a partnership between KCHFT and Kent County Council (KCC) adult social care. Our team provides short-term, personal care and rehabilitation to support patients to return home and recover in their own communities.

Key Responsibilities:
- Supporting patients to return home and recover in their own communities;
- Facilitating early supported discharge from hospital through access to health and care needs assessment at home;
- Working collaboratively with the multi-disciplinary team to ensure seamless care delivery.
Requirements:
- A full UK driving licence;
- Sound communication and interpersonal skills (verbal and written);
- Ability to organise and prioritise own work, moderate IT skills, ability to manage and prioritise care delivery in a professional manner, considering health and social care policies and procedures.
