Our mission is to empower individuals experiencing homelessness by connecting them with essential resources, housing options, and ongoing support services. We strive to create a positive and lasting impact on the lives of those we serve.
Responsibilities:- Deliver personalized, intensive outreach support to a caseload of individuals experiencing homelessness. Maintain a persistent and unwavering commitment to helping them overcome their challenges.
- Engage with clients on the streets, collaborating with partner agencies such as healthcare providers, substance abuse treatment centers, criminal justice organizations, and local authorities.
- Manage client cases effectively, maintaining accurate records of support provided, actions taken, and outcomes achieved.
- Conduct thorough initial assessments to identify individual needs and develop person-centered outcome plans that address their unique circumstances.
- Identify positive steps and achievements, no matter how small, to foster a sense of progress and motivation.
- Create dynamic risk assessments with a flexible and creative approach, tailoring strategies to each client's specific situation.
- Provide guidance and support on navigating housing pathways, advocating for their needs, and accessing essential services.
- Liaise with statutory and voluntary agencies on behalf of clients, ensuring they receive the necessary assistance and support.
- Accompany clients to appointments and interviews, providing stability and encouragement throughout the process.
- Contribute to reducing the time spent on the streets by clients, leading to decreased contact with emergency services and a safer environment.
- Offer emotional support during crisis situations, alleviating stress and anxiety for clients.
- Assist with practical skills development in areas such as managing benefits claims, shopping, attending probation meetings, and navigating healthcare appointments.
- Provide in-reach support to individuals placed in emergency accommodation, including bed & breakfast facilities, ensuring their well-being and stability.
- Participate actively in community meetings and presentations alongside managers to secure additional funding for the role and expand our services.
- Maintain accurate records, including databases, to provide data and information to commissioners and funders, demonstrating the impact of our work.
- Contribute to statistical reporting mechanisms such as street counts and responding to street link reports, providing valuable insights into homelessness trends.
- Utilize the personalization fund creatively to identify opportunities and achieve positive outcomes for clients.
- Foster a collaborative team environment by actively participating in reflective practice sessions and sharing knowledge with colleagues.
- Develop and maintain strong relationships with the local community, building trust and understanding.
- Perform any other reasonable tasks as requested by members of staff, demonstrating flexibility and willingness to contribute to the overall success of the team.
