Key Responsibilities- Undertake clinical assessments and develop treatment plans to meet the complex needs of patients requiring intermediate care and primary care services.
- Provide safe and effective care to patients, demonstrating sound clinical decision-making and expert care.
- Work collaboratively with other healthcare professionals and agencies to deliver seamless care to patients.
- Manage and support peers, working on their own initiative and as an effective team member.
- Develop and maintain knowledge and skills in clinical assessment, diagnosis, and treatment, staying up-to-date with best practices and guidelines.
- Registered Nurse Part 1,2 NMC Register or HCPC Paramedic Registration.
- A minimum of 3 years current experience practicing as band 5 in the Community/Acute environment.
- Consultation & Diagnostic qualification or working towards.
- Management of minor illnesses or injuries.
- Knowledge of admission avoidance.
- Qualified Nurse or paramedic Prescriber.
- Management education development, including in supervision/teaching/team management.
