Community Practitioner

We are seeking an experienced Registered Nurse or HCPC Paramedic to join our team as a Community Practitioner. This is an exciting opportunity to work in a dynamic and rapidly evolving environment, providing high-quality care to patients in their own homes.
Key Responsibilities- Undertake clinical assessments and develop treatment plans to meet the complex needs of patients requiring intermediate care and primary care services.
- Provide leadership and support to the team, working collaboratively with other health and social care professionals to deliver excellent patient outcomes.
- Develop and maintain strong relationships with patients, families, and carers, ensuring that their needs are met and their expectations are exceeded.
- Contribute to the development of the service, identifying opportunities for improvement and implementing changes to enhance patient care and outcomes.
- Registered Nurse Part 1,2 NMC Register or HCPC Paramedic Registration.
- A minimum of 3 years current experience practicing as band 5 in the Community/Acute environment.
- Consultation & Diagnostic qualification or working towards.
