Job Title: Staff Nurse/Primary Care Nurse
Job Type: Full-Time/Part-Time

A well-established and patient-focused medical practice in a reputable healthcare organization is seeking an experienced and dedicated Practice Nurse to join their multidisciplinary team. The ideal candidate will play a key role in delivering exceptional care to a diverse patient community.
Key Responsibilities:
- Deliver a wide range of nursing care services, including chronic disease management, wound care, immunizations, and health screenings.
- Perform patient assessments, develop individualized care plans, and ensure the delivery of patient-centered care.
- Work collaboratively with healthcare professionals, including GPs, healthcare assistants, and other members of the multidisciplinary team to ensure seamless care delivery.
- Provide patient education and health promotion, empowering patients to take an active role in their health and well-being.
- Registered Nurse (RGN) with a current NMC pin.
- Previous experience in a primary care or community setting is preferred.
- Expertise in managing chronic conditions such as diabetes, asthma, and COPD.
