We are seeking a highly motivated and experienced individual to join our team as a Community Falls Prevention Program Coordinator at ABL Health Ltd. This is an exciting opportunity to work in a fast-paced, growing community health organization that is passionate about delivering evidence-based, innovative, and effective health care services.
Key Responsibilities- Coordinate and oversee the delivery of the FaME program in Eastwood, a 24-week program that aims to improve individuals' strength and balance to increase their confidence and reduce their fear of falling.
- Support instructors in setting up and delivering the FaME program, ensuring high-quality health and wellness programs are delivered to the community.
- Deliver the existing OTAGO class in Hucknall, and set up and deliver new OTAGO classes in Arnold and Cotgrave, ensuring each runs effectively for 12 months.
- Engage with community groups to ensure effective promotion, signposting, and referrals to and from existing services, such as seated exercise, Tai-Chi, and walking groups.
- Capture accurate data from outcome measures and goal-setting targets in line with the commissioners' specifications, and monitor participant progress and provide ongoing support throughout the program.
- Prepare detailed reports and present data during performance review boards and contract reviews, and support the ongoing evaluation of this initiative as required.
- Provide one-on-one and group support to program participants, utilizing motivational interviewing and positive psychology techniques to encourage sustainable behavior change.
- Monitor and track client progress, offering personalized advice and adjustments to their wellness plans, and establish and maintain strong relationships with community stakeholders.
- Work collaboratively with the multi-disciplinary team to enhance service delivery and client outcomes, and liaise with GPs and other relevant healthcare professionals to ensure comprehensive client care.
- Maintain accurate records and data in accordance with organizational policies and procedures, and prepare and contribute to reports on program outcomes and impact.
- Ensure all activities and interventions comply with relevant guidelines and standards, and stay up to date with the latest research and best practices in health promotion and community wellness, specifically falls prevention.
- Experience in implementing and delivering community health programs, with a strong understanding of the wider determinants of health and the building blocks of health.
- Excellent written, verbal, and non-verbal communication skills, with the ability to build rapport quickly with people, and excellent administration, record keeping, and data collection skills.
- IT literate, efficient using Microsoft Office (MS Word, Excel, Outlook), and excellent time management and organizational skills.
- A proactive approach to problem-solving and program management, and the ability to work as part of a team and independently as required.
- Ability to manage stressful situations and cope in a fast-paced work environment, and willingness to undertake an Enhanced DBS check.
