About the Role

We are seeking a highly motivated and experienced Community Falls Prevention Coordinator to join our team at ABL Health Ltd. As a key member of our team, you will play a crucial role in delivering high-quality health and wellness programs, specifically focusing on falls prevention.

Key Responsibilities
  • Coordinate and oversee the delivery of the FaME program in Eastwood, ensuring its success and sustainability.
  • Support instructors in setting up and delivering the FaME program, promoting active and healthy lifestyles in the community.
  • Deliver the existing OTAGO class in Hucknall, adapting to the needs of participants and ensuring their engagement.
  • Set up and deliver new OTAGO classes in Arnold and Cotgrave, fostering a culture of health and wellness.
  • Engage with community groups to promote and signpost referrals to and from existing services, such as seated exercise, Tai-Chi, and walking groups.
  • Capture accurate data from outcome measures and goal-setting targets, monitoring participant progress and providing ongoing support.
  • Prepare detailed reports and present data during performance review boards and contract reviews, contributing to the ongoing evaluation of the initiative.
  • Provide one-on-one and group support to programme participants, utilizing motivational interviewing and positive psychology techniques to encourage sustainable behavior change.
  • Establish and maintain strong relationships with community stakeholders, working collaboratively with the multi-disciplinary team to enhance service delivery and client outcomes.
Requirements
  • Experience in implementing and delivering community health programs, with a focus on falls prevention.
  • Strong organizational and coordination skills, with attention to detail and the ability to work independently.
  • Excellent communication and interpersonal skills, with the ability to engage and motivate individuals and groups.
  • A proactive approach to problem-solving and programme management, with the ability to adapt to changing circumstances.
  • Competence in using digital tools and platforms for virtual delivery and client engagement.
