Care and Support Specialist

We are seeking a skilled Carer Support Practitioner to join our team in Shropshire Council. This role involves delivering support and information to family and unpaid carers, enabling them to achieve their own outcomes.
The successful candidate will be responsible for contributing to creating a carer-friendly community, providing expert guidance and resources to empower carers to make informed decisions about their care needs.
As a key member of our team, you will work closely with colleagues to develop and deliver tailored support plans that meet the unique needs of each individual carer.

Key Responsibilities:

• Ability to work independently and as part of a team
