Dietitian in Community Healthcare

We are seeking an experienced Dietitian to join our team as a Nutrition Support Dietitian in Central London Community Healthcare NHS Trust. As a key member of our community healthcare team, you will provide nutrition support to patients at risk of malnutrition, working closely with patients, their families, and carers to develop effective nutrition plans. With a focus on food-based approaches and appropriate prescribing of oral nutritional supplements, you will contribute to the delivery of high-quality patient care in the community. You will have the opportunity to work collaboratively with GP practices, hospital dietitians, pharmacists, and other community-based teams to ensure coordinated care. To succeed in this role, you will need skills and experience across a range of clinical dietetic specialties, gained in either an acute or community setting. A Degree / Diploma in Dietetics, Current AHP with UK HCPC registration, and Membership of the British Dietetics Association are essential.
