We are seeking a Support Practitioner to join our Helensburgh Responder Service. As a Support Practitioner, you will play a vital role in providing support to individuals in their own homes, helping them to remain safe and well.

The Responder Service is an effective and efficient way to respond to known risks that occur on an unplanned basis, such as falls and other accidents. You will work as part of a team to provide planned supports, reducing the need for ambulance call-outs and hospital admissions.

As a Support Practitioner, you will be responsible for assessing individuals' needs, developing and implementing support plans, and providing regular reviews to ensure the best possible outcomes.
