Priory Group, a leading provider of behavioural care and specialist support services, is seeking a highly skilled Clinical Integration Specialist to join our team.

The ideal candidate will be a dual-registered professional with experience in nursing and social work, holding registration with the NMC and Social Work Registration. As a Clinical Integration Specialist, you will play a key role in promoting excellent standards of patient care across our 90-bedded hospital, Kemple View.

About the Role:

  • Nursing Duties:
    • Act as the Nurse in Charge of the Ward.
    • Supervise a defined group of Nursing or HCA colleagues, ensuring they receive management and clinical supervision.
    • Prioritise nursing duties and delegate appropriately and effectively within the shift coordination framework.
    • Recognising and responding appropriately to challenging behaviour in line with company and divisional policies and training.
  • Social Work Duties:
    • Providing accurate information about care in an accessible format to patients/service users and their families.
    • Ensuring an accurate, holistic, and up-to-date social history is available for each patient.
    • Initiating visits, maintaining contact with patients' families, and offering support, information, and joint family and patient sessions where relevant.

Key Responsibilities:

  • To lead and co-coordinate the process of section 117 Mental Health Act aftercare plans with the MDT and relevant community agencies.
  • To be aware of and adhere to responsibilities for people under CPA, including offering/arranging Family and Friends carers support.
  • Liaising professionally and sensitively with other involved parties to provide seamless and consistent high-quality care for service users under care of the team.
