We are seeking a highly skilled Advanced Clinical Practitioner to join our Intensive Home Support Service (IHSS) team at East Lancashire Hospitals NHS Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality, patient-centered care to individuals with complex needs in their own homes.

IHSS is an award-winning service that aims to reduce avoidable hospital admissions and provide a comprehensive assessment and care plan in the patient’s usual place of residence. As a Clinical Practitioner, you will work closely with our team of experienced healthcare professionals to deliver care that is tailored to the individual needs of each patient.

The successful candidate will have advanced clinical skills and experience working in a variety of healthcare settings. You will be self-directed, have excellent communication skills, and be able to work effectively as part of a team. A strong understanding of the principles of governance and the ability to maintain confidentiality is essential.

Key responsibilities include:

  • Providing advanced clinical care to patients with complex needs in their own homes
  • Working closely with our multidisciplinary team to deliver care that is tailored to the individual needs of each patient
  • Conducting comprehensive assessments and developing care plans that meet the needs of each patient
  • Maintaining accurate and up-to-date patient records and contributing to quality improvement initiatives
  • Providing clinical leadership and supervision to junior team members

Key Qualifications and Experience

To be successful in this role, you will need to have:

  • A postgraduate qualification in a relevant field (e.g. Advanced Clinical Practitioner, Nurse Practitioner, etc.)
  • Significant experience working in a variety of healthcare settings, including community and acute care
  • Advanced clinical skills and experience working with patients with complex needs
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
  • A strong understanding of the principles of governance and the ability to maintain confidentiality
