Job Summary:
The role of the Clinical Pharmacist will involve working as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas. This will include many of the following responsibilities:

  • Being a prescriber, or completing training to become a prescriber, and working with and alongside the general practice team.
  • Being responsible for the care management of patients with chronic diseases and undertaking clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism.
  • Providing specialist expertise in the use of medicines while addressing both the public health and social care needs of patients.
  • Providing leadership on person-centred medicines optimisation and quality improvement.
  • Supporting patients to take their medications effectively and promoting self-care.
  • Taking a leadership role in supporting further integration of general practice with wider healthcare teams.
  • Developing relationships with other pharmacy professionals across PCNs and the wider health and social care system.
  • Taking a central role in the clinical aspects of shared care protocols and clinical research with medicines.
  • Being part of a professional clinical network.


Main Duties and Responsibilities:
The clinical pharmacy service will be primarily patient-facing and clinical in nature, focused on improving medicines optimisation and safety. The main elements include:

  • Supporting the delivery of the pharmacy and medicines optimisation elements of the network service specifications.
  • Undertaking structured medication reviews to tackle over-medication of patients.
  • Dedicated focus on priority groups such as asthma/COPD, care home residents, frail elderly, and patients with complex needs.
  • Managing medicines after discharge from hospital and ensuring continuity of care.
  • Reviewing the use of medicines most commonly associated with unplanned hospital admissions.
  • Ensuring appropriate referrals to healthcare professionals.
  • Completing the Primary care Pharmacy Education Programme if required.
  • Supporting the delivery of the Medicines Safety agenda and related quality improvements.
  • Providing a leadership role in supporting integration of general practice with healthcare teams.


Person Specification:
The ideal candidate must be enrolled in, or qualified from, an approved training pathway that equips the Clinical Pharmacist to practice and prescribe safely and effectively,

Qualifications and Further Training:
The ideal candidate must possess the following qualifications:

  • M.Pharm degree in Pharmacy (4 years vocational degree).
  • Professional registration with the General Pharmaceutical Council.
  • Independent prescriber qualification or willingness to work towards.
  • Membership of the Royal Pharmaceutical Society (RPS).


Experience:
The ideal candidate must have significant post-qualification experience, preferably in primary care and/or hospital setting,

  • Effective communication skills – written, verbal, and presentation.
  • Problem-solving and time management skills.
  • Computer literacy and use of EMIS web clinical IT system.

Job Type: Part-time

Pay: £24,000.00-£25,000.00 per year

Expected hours: 20 per week
