Care Team Coordinator

The Care Team Coordinator will be responsible for coordinating care worker visits to clients, ensuring continuity and flexibility in line with Hartwig Care's promises. This role requires effective communication with care workers, clients, and other professionals to ensure seamless service delivery.
Key responsibilities include:
- Coordinating care worker visits to reflect client needs
- Ensuring timely visits with appropriate care workers
- Updating care workers on client visits and company policies
- Receiving information/reports from care workers within set timelines
- Coordinating team meetings and review bookings
