- Contribute to the development and maintenance of relationships and connections within the local community, promoting social inclusion and engagement.">
- Collaborate with the multidisciplinary team to deliver effective support services, ensuring seamless transitions and continuity of care.">
Requirements:
">- Relevant qualifications and experience in adult care and support.">
- Excellent communication and interpersonal skills.">
- Ability to work independently and as part of a team.">
Salary Range:
">We offer a competitive salary range of £25,000 - £30,000 per annum, dependent on experience, as part of our commitment to Fair Pay, Pay Fair strategy. You will also receive paid overtime and be eligible for benefits including Wagestream, Employee Assistance Programme, and Company pension scheme.
