Job DescriptionAbout the Role

You will be working alongside a dedicated team to deliver person-centered care, using your knowledge and skills to enable our service users to live their best lives. This includes supporting them to maintain their skills, develop relationships, and access education and employment opportunities.

Main Responsibilities
  • Deliver high-quality care and support to our service users, aligned with our person-led support delivery model.
  • Engage with service users to help them maintain their skills and receive personalized care.
  • Contribute to the development and maintenance of relationships and connections within the local community.
  • Complete and maintain Support Plans and other necessary information that supports our people and ensures the delivery of effective support.
Requirements

To be successful in this role, you will need to have experience in providing support for individuals with complex needs, as well as excellent communication and interpersonal skills. You must also be able to work independently and as part of a team.
