About the Role

We are seeking a highly skilled and experienced Advanced Mental Health Practitioner to join our East Specialist Community Teams (SCT) in Berkshire. This is a fixed-term position for 18 months, working full-time hours based at Fir Tree House, Upton Hospital, Slough.

Key Responsibilities
  • Conduct comprehensive assessments and develop care plans for young people with complex mental health needs.
  • Provide therapeutic interventions and support to young people and their families.
  • Work collaboratively with multidisciplinary teams and external agencies to ensure effective care planning.
  • Manage a caseload of young people with high-risk presentations and develop risk management plans.
  • Participate in regular supervision and contribute to team professional development initiatives.

Requirements
  • Recognised and relevant qualification in mental health nursing, social work, or a related field.
  • Registration with the NMC or HCPC or other recognised professional body.
  • Post-qualifying experience in children's mental health services, including comprehensive assessments and care planning.
  • Experience of working with high-risk presentations and developing risk management plans.
  • Ability to work autonomously and effectively in a team, prioritising work and adapting to changing needs.
