The estimated salary for this position is £38,000 per annum, with an attractive bonus package based on service performance.

Your Typical Day

As a Nurse Case Manager, your day-to-day responsibilities will include:

  • Management of complex care packages, focusing on clinical and holistic approaches
  • Development, review, and implementation of care plans and risk assessments
  • Leading the clinical training needs of the care team, ensuring safe and effective practice
  • Engagement in networking, attending multidisciplinary team meetings, and working with clinical commissioning groups
  • New and ongoing competency assessments with the care staff team
  • Care package review and auditing of client notes and medication records

Requirements and Qualifications

To be successful in this role, you will need:

  • RN qualification (Child)
  • A valid NMC pin
  • Experience in high dependency or critical care settings is advantageous
  • Ability to work autonomously and flexibly
  • Excellent communication skills, both written and verbal
  • Access to a car for work-related purposes
