About the JobWe are looking for a dedicated and experienced Children's Disability Service Social Worker to join our team. As a social worker, you will be responsible for providing high-quality support to children and families affected by disability.
You will work closely with other professionals to develop and implement tailored plans that meet the unique needs of each child and family. Your expertise will help us deliver our Families 1st vision and ensure that every child has access to safe, supportive, and inclusive services.
In this role, you will have the opportunity to build strong relationships with your colleagues, partner agencies, and most importantly, the children and families you serve. If you are passionate about making a positive impact on the lives of others, we encourage you to apply.
Key Responsibilities
- Conduct comprehensive assessments to identify the needs of children and families
- Develop and implement individualized plans that promote the well-being and safety of children
- Collaborate with other professionals to provide wrap-around services and support
- Maintain accurate records and reports as required
