Community Outreach Coordinator

About the Role:
We are seeking a highly motivated and compassionate Community Liaison Officer to join our team at Home Care Direct. As a key member of our organization, you will play a vital role in promoting our Local Community Model, which empowers individuals with complex care needs to receive support from local people they trust.
Key Responsibilities:
- Develop and maintain relationships with local community groups, mosques, social groups, charities, and specialist health charities to raise awareness about our Local Community Model.
- Visit individuals who have expressed interest in receiving support from local people, fostering a sense of community and trust.
- Make presentations to local community groups, highlighting the benefits of our model and the importance of local people supporting local people.
- Collaborate with our team to identify and address the needs of individuals with complex care needs, ensuring they receive the support they require to stay out of hospitals and long-term care facilities.
- Provide financial stability for families and communities by promoting our Local Community Model.
Requirements:
- Awareness of care or care support at home would be valued.
- Excellent communication and interpersonal skills.
- High level of computer literacy, including Microsoft Publisher.
- A full driving license and own vehicle.
- Compassionate and caring attitude.
- Confidence and ability to talk to people at a professional level.
- Punctual, flexible, and willing to make necessary changes to meet any changing needs of the company.
- Willingness and flexibility to travel to various locations.
Benefits:
