Job Summary
We are seeking a highly skilled Pharmacy Technician to join our team at the Countess of Chester Hospital NHS Foundation Trust. As a Pharmacy Technician - Medication Management Specialist, you will play a vital role in ensuring the accurate and efficient management of medications for our patients.
Main Responsibilities
- Assess patients' own drugs (PODs), identify and order medications using electronic systems, and record drug histories.
- Provide patient counseling and prepare medication for discharge, ensuring a smooth transition of care.
- Work collaboratively with the pharmacy team and ward staff to support the Medicines Management service, ensuring patients receive the highest standards of care.

Requirements
- NVQ Level 3 in Pharmacy Services with appropriate underpinning knowledge.
- Registered with the GPhC and a CPD portfolio in line with GPhC requirements.
- Excellent communication and organizational skills, with the ability to work accurately and quickly under pressure.
