Support and Wellbeing Practitioner

We are seeking a motivated and empathetic individual to join our team as a Support and Wellbeing Practitioner. In this role, you will work closely with our clients and their families to identify and address their needs, providing tailored support to promote their wellbeing and independence.
As a Support and Wellbeing Practitioner, you will be responsible for:
- Conducting assessments and developing care plans
- Delivering support and interventions to promote wellbeing
- Working with clients to set and achieve goals
- Maintaining accurate records and reporting
We offer a competitive salary of £24,000-£30,000 per annum, depending on experience, plus benefits including paid mileage, early pay app request, and access to apprenticeships and nationally recognised qualifications.
