About the Role

We are seeking a skilled Community Psychiatric Nurse to join our team on a flexible shift basis. As a Community Psychiatric Nurse, you will be responsible for providing high-quality care to patients in their homes or community settings.

Key Responsibilities
  • Conduct robust assessments of patients with mental health difficulties, identifying appropriate care pathways and ensuring the least restrictive option.
  • Signpost patients to relevant care providers, including primary and tertiary care, and attend daily Red Board meetings.
  • Complete documentation and planning care for patients, presenting cases to Multi-Disciplinary Teams for discussion around care pathways.
  • Liaise with colleagues from nursing, occupational therapy, medicine, psychology, and administration to ensure comprehensive care.

Requirements
  • A minimum of 6 months paid experience as a Community Psychiatric Nurse in an UK hospital environment within the last 3 years
  • Compliance with NMC Professional Registration and current nursing practices
  • A full UK driving license and own vehicle (dependent on the Trust)
