Job Role: Renal and Community Diabetes Care Specialist

This is a dynamic role within our Diabetes team, working closely with a Diabetes Consultant and other healthcare professionals to deliver patient-centered advice and care for individuals with diabetes.

As a Renal and Community Diabetes Care Specialist, you will be expected to provide advanced assessment and management of patients' needs, and work in a domiciliary setting to empower patients to self-manage their condition.

You will be part of a well-established intermediate diabetes service, providing care to individuals with complex diabetes needs. You will also be expected to promote the health and wellbeing of people with diabetes, guiding them towards a "normal" lifestyle and reducing morbidity and premature mortality by detecting and preventing complications.

Main Responsibilities:

  • Provide patient-centered advice and care for individuals with diabetes, promoting healthy lifestyle choices and effective self-management.
  • Deliver advanced assessment and management of patients' needs, identifying and addressing complex diabetes-related issues.
  • Work collaboratively with multidisciplinary teams to provide comprehensive care and support, including education and training for patients and carers.
  • Conduct regular assessments and evaluations to ensure the effectiveness of care and make recommendations for improvement.
  • Develop and maintain strong relationships with healthcare professionals, stakeholders, and community services to facilitate seamless care pathways.

Requirements:

  • NMC registration as a Specialist Nurse
  • Postgraduate qualification in Diabetes Care or equivalent
  • Proven experience working with patients with complex diabetes needs
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a multidisciplinary team
  • Strong analytical and problem-solving skills
