Job Title: Adult Social Care Practitioner

We are seeking a highly motivated and enthusiastic individual to join our team of Social Care Practitioners at Sheffield Teaching Hospitals. As an Adult Social Care Practitioner, you will play a vital role in facilitating the safe discharge of Sheffield residents, working closely with our professional team to provide advice and practical support on a wide range of issues.

About the Role:
  • Assessing needs and managing risks of vulnerable adults
  • Supporting people with holistic and person-centred plans to achieve their goals
  • Working with people over the age of 18 who have a wide range of issues and disabilities
  • Close working with the Short Term Intervention Team and other council services
Key Responsibilities:
  • Applying the Care Act 2014 and Mental Capacity Act 2005
  • Decision-making and operating to a high level of professional autonomy
  • Providing advice and practical support to individuals and families
  • Working collaboratively with other healthcare professionals and council services
Requirements:
  • Good knowledge of prevention services and safeguarding principles
  • Ability to work with people over the age of 18 who have a wide range of issues and disabilities
  • Relevant IT skills and ability to learn new systems
  • Flexible approach and ability to work in a fast-paced environment
