Key Responsibilities
  • Provide advice and practical support to individuals on a wide range of issues, including health, housing, financial, and social isolation.
  • Work with people over the age of 18 who have a wide range of issues and disabilities, using your knowledge of prevention services and safeguarding principles.
  • Assess needs and manage risks of vulnerable adults, supporting them to achieve their goals through holistic and person-centred plans.
  • Collaborate closely with the Short Term Intervention Team and other council services to ensure effective support for individuals.
  • Operate with a high level of professional autonomy, making decisions to ensure the safety of vulnerable adults.
Requirements
  • Good knowledge of prevention services and safeguarding principles.
  • Ability to work with people over the age of 18 who have a wide range of issues and disabilities.
  • Relevant IT skills, with additional training provided for specific systems.
  • Assessment and duty experience an advantage, but a flexible approach is essential.
  • Safeguarding experience is essential.

Our work base is at the Northern General Hospital, with visits at other Sheffield Teaching Hospital sites, working with people in their own homes and out in the community.
