About the Role

We are seeking a dedicated and compassionate Care and Support Specialist to join our team. As a Reablement Support Worker, you will work alongside individuals in their homes, providing tailored support to help them regain their confidence and independence.



Key Responsibilities


  • Support individuals with daily activities such as washing, dressing, meal and drink preparation
  • Assist with ensuring medication has been taken
  • Work collaboratively with colleagues to provide a high-quality service
  • Develop strong relationships with service users, their families and other healthcare professionals
