About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Rural Warwickshire North PCN. As a Care Coordinator, you will play a vital role in providing personalized support to patients and their families, ensuring that their social and psychological needs are met.

Key Responsibilities

  • Receiving and actioning referrals from various agencies, working closely with GP practices, pharmacies, and other healthcare professionals to deliver high-quality patient care.
  • Utilizing population health intelligence to identify patients who require personalized support, and developing effective care plans to meet their needs.
  • Facilitating communication between patients, families, and healthcare providers to ensure seamless care coordination.
  • Providing navigation and coordination services to patients and their carers, ensuring they receive the support they need to manage their health.
  • Collaborating with local care homes to manage patient needs, including completing and reviewing care plans and dementia reviews as required.
  • Completing annual learning disability reviews on behalf of the practice, and undertaking assistance with clinician visits to patients in their own homes where applicable.
  • Working closely with other PCN Additional Roles staff and outside agencies to provide wrap-around support to patients.
  • Making appropriate third-party referrals, and maintaining accurate patient records.

Person Specification

  • Ability to manage and prioritize a caseload, working flexibly and enthusiastically within a team or on own initiative.
  • Excellent communication skills, both written and verbal, with the ability to build relationships with patients, families, and carers.
  • Ability to provide personalized support to individuals, their families, and carers, and to listen and empathize with people in a non-judgmental way.
  • Leadership skills, with the ability to complete tasks in a timely manner and maintain effective working relationships.
  • Ability to communicate difficult messages to patients and families, and to provide leadership and support to patients and their carers.
