Care Coordinator for Early Intervention in Psychosis

Job Description
We are seeking a skilled and compassionate Care Coordinator to join our Early Intervention in Psychosis team in North Warwickshire & Rugby. The successful candidate will be responsible for working with individuals who are experiencing the first onset of Psychosis, as well as their families.
The role involves conducting detailed initial assessments, incorporating the CAARMs assessment to identify at-risk mental states (ARMS) or first episode psychosis (FEP), and formulating a plan for the client's care over the next three years. Our team uses a Recovery philosophy to collaboratively identify the individual's goals and needs, and to support them towards achieving these.
The successful applicant will be expected to work creatively, both with other team members and independently, to support the client achieve their goals. Many of our clients experience difficulties with housing, finances, vocational activities, and social inclusion, and the successful applicant will be required to work with these issues.
About the Role
The Care Coordinator will be responsible for identifying and managing risk, utilizing the Steve Morgan tools, including positive risk taking. The successful applicant will also be expected to carry out duties as per the job description.
Person Specification
We are looking for a candidate with a relevant professional qualification, such as RMN or DipCOT, and current unrestricted registration with HCPC/NMC. The successful applicant will also have substantial experience in a Community Mental Health care setting, as well as developed specialist knowledge gained within a similar environment over a considerable period of time.
