Care Coordinator for Early Intervention in Psychosis Team

About the Role
We are seeking a skilled Care Coordinator to join our Early Intervention in Psychosis Team in North Warwickshire and Rugby. The successful candidate will work with individuals experiencing first onset of psychosis, providing holistic care and support to achieve their goals.
Key Responsibilities
- Conduct detailed initial assessments to identify at-risk mental states or first episode psychosis.
- Develop and implement care plans in collaboration with clients and their families.
- Work creatively to address housing, financial, vocational, and social inclusion challenges.
- Manage risk and utilize positive risk-taking strategies.
Requirements
- Relevant professional qualification (e.g., RMN, DipCOT).
- Current unrestricted registration with HCPC/NMC.
- Substantial experience in community mental health care settings.
- Strong IT skills and ability to maintain accurate records.
