Secure Care Rehabilitation Coordinator

Midlands Partnership NHS Foundation Trust is looking for a skilled and dedicated individual to fill the position of Secure Care Rehabilitation Coordinator. As an Occupational Therapy Assistant, you will play a crucial role in supporting the assessment, planning, and implementation of treatment plans for individuals with mental health needs. Your primary responsibility will be to prepare, organize, and carry out interventions to support the rehabilitation goals identified by occupational therapists. You will work collaboratively with the multidisciplinary team to ensure high-quality care and adapt to the demands of the ward.
The ideal candidate will possess a Quality Credit Framework qualification at level 3, relevant experience working within health and social care settings, and basic knowledge of clinical governance principles.
