We are seeking a dedicated Disability Services Coordinator to join our team at Unique Community Services. The ideal candidate will have experience supporting individuals with disabilities and working within multi-disciplinary/multi-agency environments.

About the Role

The Disability Services Coordinator will work closely with the PBS team to implement system-wide PBS practices across multiple organizations, continuously developing the service in line with the latest research and practices in the field.

Key Responsibilities
  • Providing clinical support within both community and hospital settings
  • Representing the organization at professional meetings
  • Promoting the development of individualized support packages
  • Contributing to ongoing service improvement across the wider care network
Requirements
  • Experience supporting individuals with disabilities
  • Working within multi-disciplinary/multi-agency environments
  • UK driving licence
