Key Responsibilities
  • Establish and maintain a trusting and therapeutic relationship with clients, families, and case managers.
  • Support clients in accordance with care plan guidelines, promoting their independence and well-being.
  • Monitor and maintain health, safety, and security in the workplace, ensuring a safe and supportive environment for clients.
  • Communicate effectively with all parties, including clients, families, and healthcare professionals.
  • Contribute to the protection of individuals from abuse, ensuring their dignity and respect are maintained at all times.
  • Liaise with other agencies and professionals as required, ensuring seamless care coordination.
  • Complete daily records, including time sheets, daily records, care plan summaries, and critical incidents, as advised.
  • Attend supervision, team meetings, and other meetings as nominated by the employer, ensuring ongoing professional development and support.
  • Participate in training sessions as arranged, enhancing your skills and knowledge in care and rehabilitation.
  • Support clients to attend and participate in community activities, promoting their social and emotional well-being.
  • Support clients to attend hydrotherapy sessions, as advised by treating professionals.
  • Carry out therapy practice sessions, as advised by treating professionals, to promote client progress and well-being.
Requirements
  • A minimum of an NVQ 2 or 3 in Health and Social Care or Care Certificate, or equivalent qualifications.
  • Excellent communication and interpersonal skills, backed by previous experience in supporting individuals with complex care needs.
  • Proficiency in the use of hoists and specialist equipment, ensuring safe and effective care delivery.
  • Reliability, integrity, and complete trustworthiness, essential attributes for this role.
  • A full and valid driving license, enabling you to drive a wheelchair-adapted vehicle to support client access to the community.
  • A genuine interest in providing person-centered care, with a willingness to learn and develop your skills and knowledge.
  • Successful candidates will be required to undergo an Enhanced DBS check, ensuring the highest standards of care and safety.
