Job DescriptionThis Care Coordinator role is based in the North of England and offers a competitive salary of £25,000-£26,000 per annum. As a Care Coordinator, you will be responsible for managing staff rotas, collaborating with healthcare professionals, and conducting regular assessments and reviews.
Key Responsibilities- Staff Management: Oversee staff rotas to ensure seamless delivery of care services.
- Liaise with Stakeholders: Collaborate with hospitals, GPs, social services, and families to deliver the best possible care outcomes.
- Conduct Assessments: Regularly assess and review client care plans to maintain top standards.
- Manage Rota: Oversee on-call duties and weekend work on a rota basis.
- Experience in domiciliary care is required.
- Strong IT skills and excellent organisational abilities are necessary.
- Valid driving licence and access to a vehicle.
- Proven track record in coordinating within a domiciliary care setting.
