About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Somerset Healthcare and Rehabilitation Center. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare team.

Key Responsibilities
  • Provide nursing care to patients, including administering medications and treatments as assigned.
  • Prepare for and assist physicians and other healthcare professionals with diagnostic procedures and treatments.
  • Reinforce health care instructions to patients and their families, promoting a smooth transition to home care.
  • Participate in planning, implementing, and evaluating nursing care and other patient activities, ensuring a comprehensive approach to patient care.
  • Assist with patient admissions, transfers, and discharges, maintaining a seamless experience for patients and their families.
  • Monitor and report patients' vital signs, symptoms, and general condition to the appropriate healthcare team members.
  • Record patient observations and responses to nursing care, maintaining accurate and up-to-date patient charts.
  • Maintain a safe and comfortable environment for patients, adhering to infection control and safety protocols.
  • Assume charge of the unit as assigned, demonstrating leadership and teamwork skills.
