Key Responsibilities:- Deliver high-quality, patient-focused practice nursing services to our diverse patient population.
- Assess, plan, and manage long-term conditions, health prevention, and screening programs.
- Develop, implement, and evaluate programs that promote health and well-being, and prevent adverse effects that are detrimental to health.
- Implement and evaluate individual treatment plans for patients with long-term conditions, and identify and manage patients at risk of developing a long-term condition.
- Prioritize health problems and intervene appropriately to assist patients in complex, urgent, or challenging situations.
- Support patients to adopt health promotion strategies that encourage good health, deliver opportunistic health promotion at all times, and support the principles of self-care.
- NMC registration.
- Excellent communication skills, both written and oral.
- Ability to work as part of a team and independently.
- Nursing experience in primary or secondary care.
