Older Adult Community Mental Health Nurse/Practitioner

Dorset HealthCare University NHS Foundation Trust is seeking a highly skilled and dedicated Community Mental Health Practitioner to join our team in Weymouth. As a key member of our Community Mental Health Team for Older People, you will play a vital role in providing assessment, treatment, and support to individuals suffering from organic and functional mental health illnesses.
Main Responsibilities- Assess, plan, implement, and evaluate programs of care for patients with complex mental health needs.
- Develop and maintain effective relationships with patients, carers, and other healthcare professionals to ensure seamless care coordination.
- Work collaboratively with the multidisciplinary team to provide holistic care and support to patients and their families.
- Conduct risk assessments and develop strategies to mitigate risks and promote patient safety.
- Manage a defined caseload and prioritize tasks to meet deadlines and deliver high-quality care.
