Job Summary

nLeague is seeking a highly skilled and compassionate Disease Intervention Specialist 2 to join our team as a Community Health Navigator. This role will play a critical part in our efforts to prevent substance use and promote overall health and well-being in our community.

Key Responsibilities
  • Screening and Assessment: Facilitate the screening process in participating health settings using validated tools to identify individuals at risk of substance use.
  • Counseling and Intervention: Conduct evidence-based mental health and substance use disorder assessments, provide crisis intervention, and offer solution-focused brief counseling in and outside of health department settings.
  • Education and Outreach: Provide education to prevent drug overdose deaths and adverse consequences of substance use, and conduct client outreach at non-traditional organizations and establishments associated with frequent overdoses.
  • Partnership Development: Establish partnerships with Syringe Service Providers (SSPs) and Regional Overdose Prevention Specialists (ROPS) for outreach coordination, and develop relationships with mental health agencies, drug/alcohol treatment providers, and other community agencies.
  • Record Keeping and Reporting: Maintain accurate records of client information, progress notes, and referral attainment, and submit reports as required.
Requirements
  • Education and Experience: Bachelor's degree in a related field (e.g., healthcare, social work, psychology) and one year of professional experience in healthcare for substance use at-risk populations, social work, or social or psychological counseling.
  • Certifications and Licenses: Licensed Alcohol and/or Drug Abuse Counselor (LADAC) licensure or Certified Peer Recovery Specialist (CRPS) certification desired.
  • Skills and Abilities: Excellent communication and conflict management skills, ability to work independently with minimal supervision, and high level of accountability, professionalism, and good decision-making skills.
