Disease Intervention SpecialistWe are seeking a compassionate and driven Disease Intervention Specialist to join our team. In this role, you will play a vital part in preventing drug overdose deaths and mitigating the adverse consequences of substance use within our community.
Responsibilities:- Conduct comprehensive mental health and substance use disorder assessments using evidence-based practices.
- Provide crisis intervention and solution-focused brief counseling to individuals in need, both within and outside of healthcare settings.
- Educate clients on strategies to prevent drug overdose deaths and minimize the risks associated with substance use.
- Develop strong partnerships with Syringe Service Providers (SSPs) and Regional Overdose Prevention Specialists (ROPS) to coordinate outreach efforts.
- Disseminate relevant overdose education materials to clients and the wider public.
- Connect individuals with appropriate care services, including mental health support, substance use treatment, harm reduction programs, primary healthcare, communicable disease treatment, and public health resources.
- Collaborate with a network of community agencies, such as mental health organizations, drug/alcohol treatment providers, harm reduction service providers, primary healthcare facilities, specialty healthcare providers, and hospitals.
- Maintain detailed progress notes and track referral outcomes effectively.
- Participate actively in team meetings and case conferences.
- Ensure accurate recording of client information within approved tracking systems.
- Engage in ongoing professional development by attending trainings as assigned by the organization or state requirements.
We are seeking a highly motivated and compassionate individual with a strong commitment to public health. The ideal candidate will possess:
- A Bachelor's degree in social work, psychology, public health, or a related field.
- Experience working with individuals experiencing substance use disorders and/or mental health challenges.
- Excellent communication, interpersonal, and problem-solving skills.
- A demonstrated ability to build rapport and trust with clients from diverse backgrounds.
- Strong organizational and time management abilities.
