District Nurse Clinical Specialist

The District Nurse Clinical Specialist is a key member of the Integrated Community Nursing Service, responsible for delivering and leading specialist clinical care for patients on the District Nursing caseload. This role requires a high level of clinical expertise, leadership skills, and the ability to work collaboratively with other healthcare professionals to ensure the best possible outcomes for patients.
Key Responsibilities- Assess, plan, deliver, and evaluate patient-centered programs of care, ensuring that they practice within their sphere of competence and knowledge.
- Lead and manage a team to deliver care in the home and community, promoting a culture of excellence and continuous improvement.
- Work in an integrated and partnership way with other healthcare professionals, including primary, secondary, social care, and the independent and voluntary sector, to improve the health and care of individuals, families, and communities.
- Develop and implement quality and governance agendas, ensuring that clinical practice is grounded in evidence-based theoretical and practical knowledge.
- Provide information, education, support, and advice to patients, families, carers, and healthcare professionals about aspects of care relating to Community Nursing and Locality Therapy.

Person Specification- Registered with current NMC Registration
- Hold a recognized Educational/Supervisory qualification or equivalent experience
- Specialist Practice Degree (SPQ) in District Nursing or equivalent Advanced Clinical Practice qualification
- Educated to a minimum of honors degree Level and be willing to progress to Masters Level
- Significant levels of specialist knowledge and experience, for example, in long-term conditions management, care of the elderly, Chronic Obstructive Pulmonary Disease, evidenced through short courses and/or continuous professional development.
- Independent/supplementary prescriber or commitment to undertake this course
- Proven experience at leading and managing people across disciplines and teams
- Demonstrate knowledge and skills in understanding and delivering change in service delivery
- Proven track record and experience in managing Performance and Quality
- Highly developed knowledge of Health and Social Care organizations
- Expertise in service improvement
- Proficient with a range of IT systems
- Confident at presenting complex data and information
- Demonstrate specialist clinical knowledge and expertise that promote positive changes in practice
- Maintain clinical competence
- Knowledge of the Continuing Health Care (CHC) process/personal health budgets
- Highly developed knowledge and experience of Governance and Quality assurance processes
- Highly developed interpersonal communication skills
- Physical Skills
- Physically capable of performing the role and responsibilities expected of the post holder, e.g., Moving and handling, lone working
- Car driver with clean license
- Awareness of personal safety
- Proven track record to effectively manage teams, prioritize, and delegate work
- Experience to work within an interprofessional team and ensure policies/procedures are followed
