About the Role

We are seeking a highly skilled and experienced District Nurse Clinical Specialist to join our team at Southern Health NHS Foundation Trust. As a key member of our Integrated Community Nursing Service, you will be responsible for delivering and leading specialist clinical care for patients on the District Nursing caseload.

Key Responsibilities
  • Lead and manage a team to deliver care in the home and community, developing new and innovative clinical practices in response to emerging knowledge and techniques.
  • Work in an integrated and partnership way with primary, secondary, social care, the independent and voluntary sector, and others to improve the health and care of individuals, families, and communities, particularly the most vulnerable.
  • Ensure clinical practice is grounded in evidence-based theoretical and practical knowledge, working to professional and regulatory body codes, standards, and guidance at all times.
  • Act with autonomy and authority, making decisions within an agreed professional and managerial structure, and create and promote a learning culture within community nursing to support the professional growth and development of nursing staff, students, and members of the multi-disciplinary team.
  • Provide information, education, support, and advice to patients, families, carers, and healthcare professionals about aspects of care relating to Community Nursing and Locality Therapy.
